Today, most physicians have limited “face time” with their patients. Life²’s Care Management programs serve as an excellent adjunct to physician care.
A dedicated nurse – with added support from our team of dietitians, exercise physiologists, pharmacists, health educators and other health professionals – helps members understand their condition, their doctor’s plan of care and how to become a better self-manager of their health.
Life²’s Care Management programs help members better manage:
• Asthma (pediatric and adult)
• Chronic Obstructive Pulmonary Disease
(COPD)
• Coronary Artery Disease (CAD)
• Diabetes (pediatric and adult; Types 1 & 2)
• Heart Failure (HF)
Life²’s Care Management programs also include Condition Support Programs for low back pain, kidney disease, oncology, musculoskeletal disorders and vascular at-risk members.
We start by working with the participant to create a Dynamic Participant Profile® (DPP), a personalized care “blueprint” that organizes specific goals and action steps to help achieve better health. These goals are based on an individual’s current health status and behaviors, and the physician’s prescribed treatment plan.
The primary nurse care manager then oversees an individual’s DPP by coordinating care. This nurse care manager involves other licensed HMC health care professionals when necessary, including pharmacists, dietitians, exercise physiologists and physicians.
